The responsibilities for the Hospice RN Case Manager include: conducting regular visits at home or skilled nursing facilities for a caseload of approximately 12 to 15 patients; coordinating, assessing, and implementing patient care; providing accurate and timely documentation of patient services to reflect plans of care; overseeing and participating in day to day clinical care of terminal patients and their families; attending weekly interdisciplinary meetings; interacting with other group members such as CNA, LPN, social workers and dieticians; performing physical activities that require considerable use of his/her arms and legs and moving your whole body, such as climbing, lifting, balancing, walking, stooping, and handling of biohazard materials; and participating in an on call rotation that includes 1 night per week and 1 week every 6-7 weeks.
Requirements
RN licensure (current, unrestricted) in the state location of position is required. 3 years of clinical experience is required. Reliable transportation including valid driver license and auto insurance required. Applicant with excellent communication skills, experience in long-term home care and/or hospice, computer proficiency with MS Office products including Word and Outlook and proficiency with electronic medical records (EMR) software will be preferred.
Education
certified public accountant degree required.
